Healthy Homes Coalition is recognized by the Michigan Association of Health Plans

Earlier this year, Priority Health, Spectrum Health Visiting Nurses, and Healthy Homes Coalition of West Michigan was recognized by the Michigan Association of Health Plans as a Pinnacle Award recipient in the category of Best Practices in Chronic Disease Management.

Since 2001, The Michigan Association of Health Plans has recognized health plans for achievements in addressing the challenges of a shifting healthcare environment through improvements in operations, clinical services, disease management, and community outreach.

The pilot program, namely the Asthma Environmental Housing Program, provided home assessments, education, supplies, and minor repairs to reduce asthma triggers in the home. Additionally, Spectrum Health Visiting Nurses provided assistance with managing medical concerns. Together, this collaboration of three outstanding organizations proved effective in reducing emergency room visits by 61.4% and decreased annual medical spending by $675,274.

Michigan Association of Health Plans hosted a special luncheon to highlight and celebrate all recipients of the Pinnacle Awards. Lisa McKinney of Healthy Homes Coalition of West Michigan attended the celebration, along with Kristen Burleson and Andrea Russell of Priority Health, and Tracy Alvesteffer of Spectrum Health Visiting Nurses.

The true winners are the families that learned new ways to manage asthma, resulting in improved health!
